American rapper and music executive Sean “Diddy” Combs was reportedly taken from a federal prison in New Jersey to a hospital over the weekend after suffering a head injury.

The 56-year-old musician was transported by ambulance from the Federal Correctional Institution (FCI) Fort Dix at approximately 1 a.m. on Saturday, October 10, according to a report by TMZ, which cited sources at the facility.

The report said Combs returned to the prison before 7 a.m. the same day. Sources indicated that he was conscious and able to speak when he returned.

Although the injury was described in media reports as non-life-threatening, the exact circumstances surrounding it remain unknown. TMZ also reported that it had not independently confirmed the hospital to which he was taken.

Combs’s spokesperson, Juda Engelmayer, said the rapper’s representatives were aware of the reports and were seeking to establish what happened.

“We are aware of the reports concerning Sean Combs and are working to confirm the circumstances,” the spokesperson said. “Sean’s health and well-being are our immediate concern, and we appreciate everyone who has reached out.”

Authorities at the prison had not publicly provided a detailed account of the reported incident in the reports reviewed.

The development comes as Combs continues serving a 50-month federal prison sentence following his 2025 conviction on two counts of transportation to engage in prostitution. He was acquitted of the more serious racketeering conspiracy and sex-trafficking charges brought against him.

His incarceration has attracted renewed media attention in recent weeks following reports about alleged privileges inside the prison and disciplinary action over alleged possession of contraband. Reports also said his projected release date had been moved to March 2, 2028, from January 2028.

The reported head injury has prompted questions about the circumstances of his brief removal from the facility, but there is currently no verified evidence establishing how the injury occurred or whether it resulted from an altercation.

Further details about his medical condition and the incident had not been independently confirmed as of the latest reports.
