A multiple-vehicle collision triggered by a catastrophic brake failure on a MACK flatbed truck left three people trapped in wreckage along the LASU–Isheri Road in Lagos on Tuesday, July 21, 2026, but swift intervention by operatives of the Lagos State Traffic Management Authority ensured all victims were rescued alive and rushed to hospital. The crash occurred at the Iyana School axis, inward Igando, when the articulated vehicle, reportedly travelling at speed, lost braking power, veered out of control and slammed into a Toyota Sienna and two commercial tricycles. LASTMA officials, who were on routine patrol along the corridor, immediately activated the agency's emergency response protocol upon receiving word of the accident.
According to a statement issued by the Director of Public Affairs and Enlightenment Department of LASTMA, Adebayo Taofiq, preliminary investigations confirmed that the truck suffered a catastrophic brake failure, causing the driver to lose effective control before the violent collision. The vehicles involved were identified as a MACK flatbed truck, a Toyota with registration number FGC 80 XE and a TVS commercial tricycle with registration number LSR 604 QP. Three occupants—one male and two females—sustained varying degrees of severe fractures in the collision. Through the prompt and coordinated intervention of LASTMA officers and other emergency responders, the victims were successfully extricated from the wreckage and immediately conveyed to El-Shaddai Hospital, where they are presently receiving specialised medical treatment.
LASTMA said its personnel worked alongside officers of the Iba Police Division to evacuate the damaged vehicles and clear the accident scene to prevent further crashes and traffic congestion. The agency noted that its traffic management officers remained at the scene throughout the rescue operation, directing traffic and ensuring the complete restoration of normal vehicular movement after the evacuation exercise. Reacting to the incident, the General Manager of LASTMA, Mr. Olalekan Bakare-Oki, commended the professionalism and prompt response of the agency's personnel. He also urged motorists, particularly operators of articulated and heavy-duty vehicles, to ensure their vehicles undergo regular mechanical inspections before embarking on trips. "Routine preventive maintenance of critical vehicle components remains indispensable in mitigating avoidable road traffic crashes and enhancing overall road safety across Lagos State," Bakare-Oki said. He further advised motorists to drive carefully, obey traffic regulations and promptly report road emergencies through LASTMA's toll-free emergency short code, 3367, to enable the agency's emergency response teams to intervene quickly.
