Two Frankfurt Airport Workers Die After Rare Malaria Outbreak Linked to Imported Mosquito

Two workers at Germany’s Frankfurt Airport have died after contracting malaria in an unusual outbreak that health authorities believe was caused by infected mosquitoes transported into the country aboard an aircraft. The incident has drawn attention because locally acquired malaria is exceptionally rare in Germany.

Frankfurt’s health authority confirmed that six people working at the airport have been diagnosed with malaria tropica since early July. Two of those employees have died, while the other four received hospital treatment and have been discharged. The first death occurred in early July, although malaria was identified as the cause only after the worker had died. A second employee died in August, bringing the confirmed death toll to two.

The cases have been classified as so-called airport malaria, a rare form of transmission in which an infected mosquito is transported from an endemic area by aircraft and later bites people at or near an airport. German health officials say an imported Anopheles mosquito is the most likely source of the Frankfurt infections, although investigators are still working to establish the precise species and origin of the insect.

The outbreak became public in July, when Germany’s Robert Koch Institute, the country’s national public health institute, reported several malaria cases among Frankfurt Airport employees who had become ill between July 4 and July 6. As additional infections were identified and the first death was investigated, authorities expanded the reported total to six.

The timing and circumstances of the illnesses have strengthened the theory that the infections were acquired at the airport rather than during travel. The affected employees had no reported travel history to malaria-endemic areas that would otherwise provide a more conventional explanation. Authorities have nevertheless described the mosquito link as the likely explanation rather than a conclusively established source.

Frankfurt’s public health department said the six cases involved malaria tropica, caused by Plasmodium falciparum and regarded as the most dangerous human malaria parasite. The disease is transmitted by infected female Anopheles mosquitoes. It does not ordinarily spread from person to person through casual contact, and the Frankfurt health authority has said there is no evidence that the outbreak creates an elevated risk for the general population.

Malaria can initially resemble other common illnesses. Fever, chills and headache are among its typical early symptoms, while severe infection can lead to complications affecting the brain and other organs. The World Health Organization stresses that prompt diagnosis and treatment are critical, particularly for P. falciparum malaria, which can progress rapidly to severe disease and death if treatment is delayed.

The Frankfurt cases also illustrate a challenge associated with malaria in countries where the disease is not normally transmitted. Because clinicians are more likely to associate malaria with recent travel, an infection acquired locally can initially be difficult to recognize. German authorities have therefore urged airport employees who develop possible malaria symptoms to tell their doctors that they work at Frankfurt Airport and that airport-associated malaria cases have been recorded.

Fraport, the company that operates Frankfurt Airport, has informed employees about the outbreak and encouraged them to seek medical attention if they experience symptoms. The operator has also expressed its condolences to the families and friends of the workers who died.

Authorities have begun mosquito monitoring at the airport as part of the investigation. Traps have been installed to capture mosquitoes in and around relevant areas. Specimens collected during the monitoring programme are being examined, including genetic analysis intended to determine their species and potentially establish where they originated. Investigators are also carrying out genetic examinations of the malaria parasites to assess whether the infections are linked.

Frankfurt is Germany’s busiest airport and a major international aviation hub, with extensive connections to countries where malaria remains endemic. A single infected mosquito surviving a flight can, in rare circumstances, create an opportunity for transmission after arrival if it reaches a suitable environment and finds people to bite.

The episode is not the first instance of airport malaria recorded in Frankfurt. German public health authorities said the last known case in the country occurred at Frankfurt Airport in 2023. They are considered distinct from sustained local malaria transmission because the infections originate from an imported mosquito rather than an established population of malaria-carrying mosquitoes circulating within the local community.

Health authorities have sought to reassure travellers and residents that the current situation does not represent a general malaria outbreak in Frankfurt. The city’s health department said there is currently no increased health risk for the wider population. That assessment reflects the fact that malaria is not transmitted through ordinary human contact and that the suspected mechanism requires an infected mosquito capable of transmitting the parasite.

Nevertheless, the deaths have highlighted the importance of surveillance around major airports, particularly those receiving flights from malaria-endemic regions. Airport authorities use measures designed to prevent insects from being transported between countries, while public health agencies monitor unusual infections and investigate possible imported vectors. The Frankfurt investigation may provide further information about whether existing controls need to be strengthened.

The World Health Organization estimates that 282 million people contracted malaria in 2024 and about 610,000 died. Most cases and deaths occurred in Africa.

The Frankfurt outbreak is therefore unusual not because malaria itself is unfamiliar, but because of the setting and route of transmission. In a country where malaria is generally imported by travellers, six workplace-associated infections and two deaths linked to a suspected aircraft-borne mosquito represent an exceptional public health event.

German authorities continue to investigate how the mosquitoes entered the airport, whether a specific aircraft or route can be identified, and whether additional infected insects remain present. For now, officials are emphasizing surveillance, early medical assessment and mosquito monitoring while urging the public not to confuse this rare event with widespread community transmission.

At Frankfurt Airport, the investigation now focuses on identifying the precise source of the infections and ensuring that any remaining risk is contained.
